H1. Chronic Conditions Guide and Carers

The Health, Wellbeing and Attendance Policy was ratified in August 2014 and includes information on support available for staff with chronic conditions.
Managing sickness absence masterclasses were arranged throughout the year using a multidisciplinary approach of Human Resources, Physiotherapy, Occupational Health and an experience manager who is part of the health and wellbeing group to provide advice, information, case studies outlining practical solutions to help managers with an employee with a chronic condition remain at work.
The health and wellbeing strategy group has arranged a long term health issues action group which meets on a monthly basis.
The Ipswich Hospital NHS Trust continues to provide a range of support service for all staff with chronic conditions which includes occupational health, CARE confidential listening service, counselling sessions and CBT.
Spiritual support is also available form the chaplaincy service and they have became part of the health and wellbeing strategic group.
All staff are able to self refer to the on site occupational health physiotherapy serivce for assessment, treatment and assistance in managing chronic conditons.
On site occupational health department offers advice to both managers and employees on reasonable adjustments or restrictions required to help an employee remain at or return to work.
Managers and employess are encouraged to ensure that regular communication in maintained during periods of sickness absence, this ensures that the employer and employee are both well informed about any support measures or onward refrrals required to enable the employee to return to work. This is followed up on the return to work by an interview to discuss the any ongoing health concerns and the support measures in place.
The trust has a health and wellbeing intranet site which signposts to a number of support services e.g NHS choices and Mindful Employer. The site has been further developed over the last 12 months.
