H1. Chronic Conditions Guide and Carers

Staff with chronic conditions are supported according to individual need through Occupational Health and, where appropriate, through referral to external schemes or programmes. Conditions managed by HR teams across our sites include both physical and mental health issues.
In June 2014 we published our Health and Safety Handbook which sets out our procedure around Occupational Health. This includes a risk assessment, pre- and post-employment screening and medicals, health surveillance, rehabilitation, health promotion and record keeping.
Staff with chronic conditions are helped with rehabilitation programmes, access to Occupational Health assessments, physiotherapy if required, restricted duties, either permanent or temporary, and access to health professionals.
Staff with health issues are monitored by Occupational Health on a regular basis whether they are at work or off work. Our HR teams work closely with local GPs to support individuals back to work or to help them remain at work. Referrals to external agencies are made where appropriate and have included amongst others:

- Alzheimer's Society
- British Heart Foundation
- The Stroke Association
- Arthritis/Cancer research
- Depression Alliance
- Mental Health Foundation
- Diabetes UK
