H1. Chronic Conditions Guide and Carers

Please describe how you have supported staff with chronic conditions or those with unpaid caring responsibilities over the last year. There is a 2000-character limit when completing this section
During 2014/2015 we have updated our absence policy and procedures utilising the Responsibility Deals Chronic Conditions Guide and have also included information regarding support for carers of colleagues with Chronic Conditions in line with the updated pledge. The Chronic Conditions Guide has also been sent and briefed to our team of HR Advisors who take calls from our Unit Managers to ensure they are providing the right levels of advice and support. We are also in the process of developing a new handbook which all colleagues will have access to and have included information from the Chronic Conditions Guide into the text. We also intend to launch a Managers guide in the summer of 2015.
Also in 2014/2015 we have launched and embedded our colleagues Health and Wellbeing programme - My Health and Wellbeing which is focussed on both keeping colleagues well, preventing the onset of chronic conditions as well as providing support and advice to help colleagues return to work after an illness. The objectives of the programme are to:
1. Provide support and education to our colleagues on a wide range of health related areas.
2. Encourage behavioural changes to help colleague lead a healthier lifestyle
3. Provide Support Services to colleagues when they need it
4. Build a communications platform to communicate H&W benefits effectively
The My Health and Wellbeing programme contains many services to support colleagues including: Employee Assistance Programme with 24 hour 365 day counselling support, Health Coach, Health Risk Assessment, I-Will health resources, Calendar of health education and 3 yearly health challenges. The health calendar has covered many topics from hydration to healthy eating to financial concerns to sleep management and much more. The health challenges have been focused on Physical Resilience and Losing Weight and we are just starting our third challenge which is focussed on improving the activity levels of colleagues. Our 4 week weight loss challenge led to an average weight loss of 2.5kg for each colleague who took part in the challenge with the challenge winner losing an impressive 5.5kg. 98% of colleagues reported that they will continue with the changes they have made during the challenge.
